Worth knowing

  • The welcome screen and the intro text above the first question carry example turnaround times, policies and promises. Rewrite them as yours before you publish — they are the part respondents take literally.

How to use this template

  1. Open it as a draft

Press "Use this template" and it lands in your workspace as an editable form with all 7 questions already in it. No card, and nothing is published until you say so.

  2. Cut what you do not need

Every question you remove is one fewer reason to abandon the form. It takes about 2.5 minutes to fill in as it stands, and that is the number worth protecting.

  3. Add from the question bank

Pick what you are missing from the questions below. Drag any of them in, change the wording, and use conditional logic so people only see the ones that apply to them.

  4. Share it, or put it on your own site

Publish to get a link, or embed it in a page you already have. Answers arrive in your dashboard, and you can send them on to anywhere else with a webhook or an integration.

32 quote request form questions to choose from

A quote request form fails when it collects too little to price anything, and then everybody spends a week on email filling the gaps. Ask the questions that change the number.

If your pricing is a formula, the calculator template gives people the figure on the spot instead.

The job

  • What do you need a quote for?
  • Describe the job in a few sentences.
  • What size or quantity are we talking about?
  • Where is the work or delivery?
  • When do you need it by?
  • Is the date fixed or flexible?
  • Is this a one-off or ongoing?
  • Has anyone quoted for this already?

Specifications

  • What specification or materials do you need?
  • Are there any standards it has to meet?
  • Do you have drawings, measurements or a brief?
  • Attach anything that would help us price it.
  • Are there options you would like priced separately?
  • What is essential and what is nice to have?
  • Is there an existing setup we would work with?
  • Is access or installation included?

Budget and decision

  • Do you have a budget range?
  • Is the budget approved?
  • Who signs this off?
  • How many quotes are you getting?
  • What matters most: price, speed or quality?
  • When will you decide?
  • Are there payment terms you need?
  • Would you want it broken down by line item?

Contact

  • What is your name?
  • What is your company?
  • What is your email address?
  • What is the best number to reach you?
  • What is the delivery or site address?
  • Who is the day-to-day contact?
  • How would you like to receive the quote?
  • Is there anything else we should know?

Quote Request form questions, answered

What should a quote request form ask?
Whatever changes the price, and nothing else. Quantity, specification, location and deadline usually do; job title usually does not.
Should I show prices on the form instead?
If your pricing is a formula, yes. A calculator that returns a number immediately converts far better than a form that promises one within two working days, and it filters out the people who were never going to afford it.
How do I handle file uploads on a quote form?
Add a file upload question and set the accepted types and size. Drawings, photos and specification documents answer more pricing questions than any amount of free text.
Can I take a deposit at the same time?
Yes, if the price is known. A payment block can collect a fixed deposit or a calculated amount in the same form, which turns an enquiry into a commitment.
Is this quote request form template free?
Yes. Use it, publish it and collect answers on the free plan with no card. Paid plans add things like payments, scheduling, removing our badge and your own domain, but none of them are needed to run this form.
Do I need an account to use it?
You need one to edit and publish, because the form has to live somewhere and the answers have to come back to you. Signing up takes about a minute and does not ask for a card. The people filling your form in never need an account.
Can I change the questions?
All of them. The template is a starting point, not a fixed form: add, remove, reword and reorder anything, change the look, and use logic so people only see the questions that apply to them.
Can I put it on my own website?
Yes. Embed it in a page you already have, or share the link. On a paid plan you can serve it from your own domain so the address says your name rather than ours.
Where do the answers go?
Into your dashboard, where you can read, search and export them as CSV. You can also have each response emailed to you, pushed to another tool with a webhook, or sent to a connected integration.
How long does this form take to fill in?
About 2.5 minutes as it stands, across 7 questions. Every question you cut brings that down, and shorter forms get finished more often.
