How to use this template
Open it as a draft
Press "Use this template" and it lands in your workspace as an editable form with all 4 questions already in it. No card, and nothing is published until you say so.
Cut what you do not need
Every question you remove is one fewer reason to abandon the form. It takes about 1 minutes to fill in as it stands, and that is the number worth protecting.
Add from the question bank
Pick what you are missing from the questions below. Drag any of them in, change the wording, and use conditional logic so people only see the ones that apply to them.
Share it, or put it on your own site
Publish to get a link, or embed it in a page you already have. Answers arrive in your dashboard, and you can send them on to anywhere else with a webhook or an integration.
32 lead capture form questions to choose from
Every field on a lead capture form trades volume for qualification. There is no universally right number, only the right number for what happens next: if a human calls every lead, qualify hard; if it feeds a newsletter, ask for an email and stop.
The groups below are roughly in order of what they cost you in conversion.
The minimum
- What is your email address?
- What is your name?
- What company are you with?
- What is your role?
- What is your phone number?
- Which country are you in?
- What is your company website?
- How did you hear about us?
What they need
- What are you trying to solve?
- What are you using today?
- What is not working about it?
- Which of our products are you interested in?
- What made you look now?
- What would success look like in six months?
- Is there a specific feature you need?
- Is anything an absolute requirement?
Fit and size
- How many employees does your company have?
- How many people would use this?
- Which industry are you in?
- Do you have a budget range in mind?
- What is your timeline?
- Are you the decision maker?
- Who else is involved?
- Are you evaluating other options?
Next step
- Would you like a demo?
- Would you rather start a free trial?
- What is the best time to reach you?
- Would you prefer email or a call?
- Can we add you to our newsletter?
- Would a case study for your industry help?
- Is there a question we can answer right now?
- Anything else we should know?
Lead Capture form questions, answered
- How many fields should a lead capture form have?
- Three to four if volume matters, six to eight if a salesperson will call every one. Each additional field costs conversion, so the question is whether the qualification it buys is worth more than the leads it loses.
- Should I ask for a phone number on a lead form?
- Only if you will actually call. A phone field is one of the most expensive you can add in conversion terms, and most teams that add it never ring anyone.
- How do I qualify leads without a long form?
- Ask two or three questions that genuinely change what you do next, and use conditional logic to ask the rest only of people who look like a fit. A scored quiz is a gentler way to collect the same information.
- Can I send leads straight to my CRM?
- Yes. A webhook or a connected integration creates the record where your sales team already works, so nothing sits in a form dashboard waiting to be exported.
- Is this lead capture form template free?
- Yes. Use it, publish it and collect answers on the free plan with no card. Paid plans add things like payments, scheduling, removing our badge and your own domain, but none of them are needed to run this form.
- Do I need an account to use it?
- You need one to edit and publish, because the form has to live somewhere and the answers have to come back to you. Signing up takes about a minute and does not ask for a card. The people filling your form in never need an account.
- Can I change the questions?
- All of them. The template is a starting point, not a fixed form: add, remove, reword and reorder anything, change the look, and use logic so people only see the questions that apply to them.
- Can I put it on my own website?
- Yes. Embed it in a page you already have, or share the link. On a paid plan you can serve it from your own domain so the address says your name rather than ours.
- Where do the answers go?
- Into your dashboard, where you can read, search and export them as CSV. You can also have each response emailed to you, pushed to another tool with a webhook, or sent to a connected integration.
- How long does this form take to fill in?
- About 1 minutes as it stands, across 4 questions. Every question you cut brings that down, and shorter forms get finished more often.

- Qualifying leads without running an interrogationEvery question that filters out a bad lead also loses a few good ones. Where that trade is worth making, and where it quietly is not.
