The format popularised by Typeform: a single question fills the screen, answering moves you forward, and the whole thing feels like a conversation rather than paperwork. On longer forms it reliably gets more people to the end.

One question on screen means no scanning, no scrolling, and no sense of a long form ahead. People answer what's in front of them.
Fully keyboard driven
Enter to continue, letter keys to pick a choice, arrows to move between options. Fast for anyone who prefers not to reach for a mouse.
Motion that isn't annoying
Transitions are quick and purposeful, and they respect prefers-reduced-motion — if someone's system asks for less movement, they get it.
Accessible, genuinely
Focus moves to each new question, changes are announced to screen readers, and every control is reachable by keyboard. Conversational forms are usually where accessibility breaks; ours is built not to.
