Net Promoter Score

Zero to ten, then one open question. The scale is fixed at the standard range on purpose — a rescaled NPS can't be compared with anyone else's, which is the only reason to run one.

How to use this template

  1. Open it as a draft

Press "Use this template" and it lands in your workspace as an editable form with all 2 questions already in it. No card, and nothing is published until you say so.

  2. Cut what you do not need

Every question you remove is one fewer reason to abandon the form. It takes about 1 minutes to fill in as it stands, and that is the number worth protecting.

  3. Add from the question bank

Pick what you are missing from the questions below. Drag any of them in, change the wording, and use conditional logic so people only see the ones that apply to them.

  4. Share it, or put it on your own site

Publish to get a link, or embed it in a page you already have. Answers arrive in your dashboard, and you can send them on to anywhere else with a webhook or an integration.

32 nps survey form questions to choose from

Net Promoter Score is one question and a follow-up, and almost every team that runs it adds enough extra questions to ruin the response rate it was chosen for. The score is useful as a trend, not as a number you report once.

Below: the standard question, the follow-ups that make it actionable, and the segmentation that tells you whose score moved.

The score

  • How likely are you to recommend us to a friend or colleague? (0 to 10)
  • What is the main reason for your score?
  • What would it take to move that up by one point?
  • How likely are you to still be with us in a year? (0 to 10)
  • How likely are you to buy from us again?
  • Has your opinion of us changed recently?
  • If it has, what changed it?
  • Who would you recommend us to, specifically?

For promoters (9 to 10)

  • What do we do better than anyone else?
  • What was the moment you realised this was working?
  • Would you be willing to leave a public review?
  • Can we quote you by name?
  • Would you talk to a prospective customer for us?
  • Is there anyone you have already recommended us to?
  • What should we never change?
  • Would you be interested in our referral programme?

For passives and detractors (0 to 8)

  • What is the single biggest thing we could fix?
  • What did you expect that you did not get?
  • Was there a specific moment that went wrong?
  • Have you contacted us about it?
  • What are you using instead, if anything?
  • What would bring you back?
  • Would you like someone to get in touch?
  • Is there anything stopping you leaving?

Segmenting the score

  • How long have you been a customer?
  • Which plan are you on?
  • How often do you use it?
  • What is your role?
  • How big is your team?
  • Which product do you use most?
  • How did you first find us?
  • Who else at your organisation uses it?

NPS Survey form questions, answered

What is a good NPS score?
Anything above zero means more promoters than detractors, and above 50 is genuinely strong. But the absolute number is less useful than the direction: compare yourself last quarter, not an industry benchmark gathered under different conditions.
How is NPS calculated?
Percentage of promoters (9 to 10) minus percentage of detractors (0 to 6). Passives (7 to 8) count toward the total but not the score. The range is -100 to +100.
How many questions should an NPS survey have?
Two: the score and why. Adding a third costs you response rate, and the reason is where every bit of the usable information lives. If you need segmentation, take it from data you already hold rather than asking.
How often should I run an NPS survey?
Quarterly for a trend, or triggered after a meaningful event. Monthly surveys to the same list train people to ignore you, and the score moves less than the noise in your sample.
Is this nps survey form template free?
Yes. Use it, publish it and collect answers on the free plan with no card. Paid plans add things like payments, scheduling, removing our badge and your own domain, but none of them are needed to run this form.
Do I need an account to use it?
You need one to edit and publish, because the form has to live somewhere and the answers have to come back to you. Signing up takes about a minute and does not ask for a card. The people filling your form in never need an account.
Can I change the questions?
All of them. The template is a starting point, not a fixed form: add, remove, reword and reorder anything, change the look, and use logic so people only see the questions that apply to them.
Can I put it on my own website?
Yes. Embed it in a page you already have, or share the link. On a paid plan you can serve it from your own domain so the address says your name rather than ours.
Where do the answers go?
Into your dashboard, where you can read, search and export them as CSV. You can also have each response emailed to you, pushed to another tool with a webhook, or sent to a connected integration.
How long does this form take to fill in?
About 1 minutes as it stands, across 2 questions. Every question you cut brings that down, and shorter forms get finished more often.
