General knowledge quiz
A working quiz: each right answer is worth a point, the score adds up as the respondent answers, and the ending they land on depends on how they did. Replace the questions and the scoring keeps working.

How to use this template
Open it as a draft
Press "Use this template" and it lands in your workspace as an editable form with all 5 questions already in it. No card, and nothing is published until you say so.
Cut what you do not need
Every question you remove is one fewer reason to abandon the form. It takes about 1.5 minutes to fill in as it stands, and that is the number worth protecting.
Add from the question bank
Pick what you are missing from the questions below. Drag any of them in, change the wording, and use conditional logic so people only see the ones that apply to them. Scoring is already set up, so new questions can feed it.
Share it, or put it on your own site
Publish to get a link, or embed it in a page you already have. Answers arrive in your dashboard, and you can send them on to anywhere else with a webhook or an integration.
32 quiz form questions to choose from
A quiz that scores itself is a different thing from a form with right answers in a spreadsheet afterwards. The scoring is what makes it shareable, and the result screen is what makes it spread.
Below: question formats that work, and the structural decisions that matter more than the questions themselves.
Question formats that score cleanly
- Multiple choice with one correct answer
- True or false
- Pick all that apply (partial credit, or all-or-nothing)
- Fill in the missing word
- Put these in the right order
- Match the pair
- Which of these does not belong?
- Estimate a number, scored by how close they get
Setting it up
- How many questions? (Seven to twelve finishes best)
- Is there a time limit?
- Can people go back and change an answer?
- Do you show the right answer after each question or at the end?
- Do all questions carry the same weight?
- Is there a pass mark?
- Can people retake it?
- Are the questions in a fixed order or shuffled?
The result screen
- What score did they get, out of what?
- What does that score mean? (A band, not just a number)
- Which ones did they get wrong?
- What should they read or do next?
- Can they share the result?
- Can they challenge a friend?
- Do they get a certificate or a badge?
- Can they see how they compare to others?
Who took it
- What is your name?
- What is your email address, if you want your result sent?
- Where are you from?
- How did you hear about this quiz?
- How confident did you feel before starting?
- How hard did you find it?
- Which question was hardest?
- Would you like more quizzes like this?
Quiz form questions, answered
- How do I make a quiz that scores automatically?
- Add a score variable, give each correct answer a value, and route to a different ending screen depending on the total. The whole thing is set up in the builder with no formulas to write and no spreadsheet afterwards.
- How many questions should a quiz have?
- Seven to twelve for something people do for fun. Completion falls away sharply past fifteen, and a quiz nobody finishes scores nobody.
- Can I show a different result depending on the score?
- Yes. Outcome routing sends people to different ending screens based on their score, so a top result and a poor one get different messages and different next steps.
- Should I ask for an email before or after the quiz?
- After. Asking first costs you most of the people who would have finished, and somebody who has just invested two minutes is far more likely to hand over an address to see their result.
- Is this quiz form template free?
- Yes. Use it, publish it and collect answers on the free plan with no card. Paid plans add things like payments, scheduling, removing our badge and your own domain, but none of them are needed to run this form.
- Do I need an account to use it?
- You need one to edit and publish, because the form has to live somewhere and the answers have to come back to you. Signing up takes about a minute and does not ask for a card. The people filling your form in never need an account.
- Can I change the questions?
- All of them. The template is a starting point, not a fixed form: add, remove, reword and reorder anything, change the look, and use logic so people only see the questions that apply to them.
- Can I put it on my own website?
- Yes. Embed it in a page you already have, or share the link. On a paid plan you can serve it from your own domain so the address says your name rather than ours.
- Where do the answers go?
- Into your dashboard, where you can read, search and export them as CSV. You can also have each response emailed to you, pushed to another tool with a webhook, or sent to a connected integration.
- How long does this form take to fill in?
- About 1.5 minutes as it stands, across 5 questions. Every question you cut brings that down, and shorter forms get finished more often.
