Registration & RSVP

Event registration

Everything a venue will ask you for. Dietary requirements and accessibility are asked as open text rather than a tick list, because a tick list always misses someone.

Worth knowing

  • The welcome screen and the intro text above the first question carry example turnaround times, policies and promises. Rewrite them as yours before you publish — they are the part respondents take literally.

How to use this template

  1. Open it as a draft

Press "Use this template" and it lands in your workspace as an editable form with all 7 questions already in it. No card, and nothing is published until you say so.

  2. Cut what you do not need

Every question you remove is one fewer reason to abandon the form. It takes about 2 minutes to fill in as it stands, and that is the number worth protecting.

  3. Add from the question bank

Pick what you are missing from the questions below. Drag any of them in, change the wording, and use conditional logic so people only see the ones that apply to them.

  4. Share it, or put it on your own site

Publish to get a link, or embed it in a page you already have. Answers arrive in your dashboard, and you can send them on to anywhere else with a webhook or an integration.

32 event registration form questions to choose from

Event registration has to collect enough to run the event and little enough that people finish signing up. Dietary requirements and accessibility needs belong on the form; job title usually does not.

Ask the accessibility question. It is cheap, and leaving it out means somebody finds out on the day.

Who is coming

  • What is your name?
  • What is your email address?
  • What is your phone number?
  • What organisation are you with?
  • What is your job title?
  • How should your name appear on your badge?
  • Is this your first time at one of our events?
  • How did you hear about it?

Attendance details

  • Which days will you attend?
  • Which sessions would you like to join?
  • Are you attending in person or online?
  • Will you be at the evening event?
  • Are you bringing a guest?
  • What is their name?
  • Do you need a parking space?
  • Will you need accommodation information?

Access and dietary needs

  • Do you have any dietary requirements?
  • Do you have any allergies we should know about?
  • Do you have any accessibility requirements?
  • Would you like a seat near an exit or the front?
  • Do you need a BSL interpreter or captioning?
  • Is there anything that would make the day easier for you?
  • Who should we contact in an emergency?
  • What is their number?

Payment and admin

  • Which ticket type do you need?
  • Do you have a discount code?
  • Is your organisation paying or are you?
  • Do you need an invoice?
  • What are the billing details?
  • Do you agree to the terms and the cancellation policy?
  • May we share your name with other attendees?
  • May we photograph the event?

Event Registration form questions, answered

What should an event registration form ask?
Name, contact, which sessions, and dietary and accessibility requirements. Everything else is optional, and the accessibility question is the one people regret leaving out.
Can I take payment for event tickets in the form?
Yes. A payment block takes card or PayPal in the same submission, with fixed prices per ticket type or an amount worked out from the answers.
How do I limit the number of registrations?
Set a response cap in the form's settings and it closes itself when it is reached, with a message you write rather than a broken form.
Can registrants get a confirmation email?
Yes, with a summary of their answers, and you can attach a calendar file so the date lands in their diary rather than their inbox.
Is this event registration form template free?
Yes. Use it, publish it and collect answers on the free plan with no card. Paid plans add things like payments, scheduling, removing our badge and your own domain, but none of them are needed to run this form.
Do I need an account to use it?
You need one to edit and publish, because the form has to live somewhere and the answers have to come back to you. Signing up takes about a minute and does not ask for a card. The people filling your form in never need an account.
Can I change the questions?
All of them. The template is a starting point, not a fixed form: add, remove, reword and reorder anything, change the look, and use logic so people only see the questions that apply to them.
Can I put it on my own website?
Yes. Embed it in a page you already have, or share the link. On a paid plan you can serve it from your own domain so the address says your name rather than ours.
Where do the answers go?
Into your dashboard, where you can read, search and export them as CSV. You can also have each response emailed to you, pushed to another tool with a webhook, or sent to a connected integration.
How long does this form take to fill in?
About 2 minutes as it stands, across 7 questions. Every question you cut brings that down, and shorter forms get finished more often.
