Contact form

A short contact form: who you are, how to reach you, and what you want. The subject is a choice rather than a free-text line so replies can be routed without anyone reading them first.

Worth knowing

  • The welcome screen and the intro text above the first question carry example turnaround times, policies and promises. Rewrite them as yours before you publish — they are the part respondents take literally.
  • Change the subject options to match who's actually on the other end.
  • Turn on the email automation so the sender gets a receipt.

How to use this template

  1. Open it as a draft

Press "Use this template" and it lands in your workspace as an editable form with all 4 questions already in it. No card, and nothing is published until you say so.

  2. Cut what you do not need

Every question you remove is one fewer reason to abandon the form. It takes about 1.5 minutes to fill in as it stands, and that is the number worth protecting.

  3. Add from the question bank

Pick what you are missing from the questions below. Drag any of them in, change the wording, and use conditional logic so people only see the ones that apply to them.

  4. Share it, or put it on your own site

Publish to get a link, or embed it in a page you already have. Answers arrive in your dashboard, and you can send them on to anywhere else with a webhook or an integration.

32 contact form questions to choose from

A contact form has one job: get the message to the right person with enough context that the reply can be useful. Most of them fail at the second half, which is why so many replies start by asking what the first message should have.

The template below is the short version. The questions after it are what you add when a plain message box is not sorting your inbox fast enough.

Who is getting in touch

  • What is your name?
  • What is the best email address to reply to?
  • What company or organisation are you with?
  • What is your role there?
  • Which country are you in?
  • How did you hear about us?
  • Is this your first time contacting us?
  • What is your website, if you have one?

What they want

  • What is this about? (Sales, support, billing, press, something else)
  • Tell us what you need in a sentence or two.
  • Which product or service is this about?
  • Is this about an existing order or account?
  • What have you already tried?
  • What would a good outcome look like for you?
  • Is there a deadline we should know about?
  • Is anyone else at your end involved in this?

How urgent it is

  • How soon do you need an answer? (Today, this week, no rush)
  • Is anything blocked right now because of this?
  • Is this affecting one person or your whole team?
  • Would a phone call be faster than email?
  • What times are you usually free to talk?
  • Which time zone are you in?
  • Is there a better number to reach you on?
  • Would you rather we replied by email only?

Making the reply better

  • Attach anything that would help: a screenshot, a document, a link.
  • Which browser and device are you using?
  • If this is about a page on our site, which one?
  • Is there anything we should not put in writing?
  • Would you like a copy of this message sent to you?
  • Can we add you to our mailing list? (Optional, and separate)
  • Is there anything else we have not asked about?
  • How would you prefer we address you?

Contact form questions, answered

What should a contact form ask for?
A name, a reply address, and enough about the request that whoever picks it up can answer without a second round trip. Everything beyond that is a trade: each extra field sorts your inbox slightly better and costs you a few percent of the people who would have written in.
How many fields should a contact form have?
Three to five for a general contact form. If you need more than that, it is usually a sign you have two different forms wearing one name: a sales enquiry and a support request want different questions, and splitting them beats adding fields to both.
How do I stop spam on a contact form?
Turn on the spam challenge in the form's settings. It is a Cloudflare Turnstile check that most people never have to click, and it stops the automated submissions that make a contact inbox unusable. A honeypot field runs on every form regardless.
Can I send contact form submissions to my email?
Yes. Set up a notification so every submission arrives in your inbox, and send an automatic acknowledgement to the person who wrote in so they know it landed.
Is this contact form template free?
Yes. Use it, publish it and collect answers on the free plan with no card. Paid plans add things like payments, scheduling, removing our badge and your own domain, but none of them are needed to run this form.
Do I need an account to use it?
You need one to edit and publish, because the form has to live somewhere and the answers have to come back to you. Signing up takes about a minute and does not ask for a card. The people filling your form in never need an account.
Can I change the questions?
All of them. The template is a starting point, not a fixed form: add, remove, reword and reorder anything, change the look, and use logic so people only see the questions that apply to them.
Can I put it on my own website?
Yes. Embed it in a page you already have, or share the link. On a paid plan you can serve it from your own domain so the address says your name rather than ours.
Where do the answers go?
Into your dashboard, where you can read, search and export them as CSV. You can also have each response emailed to you, pushed to another tool with a webhook, or sent to a connected integration.
How long does this form take to fill in?
About 1.5 minutes as it stands, across 4 questions. Every question you cut brings that down, and shorter forms get finished more often.
