How to use this template

  1. Open it as a draft

Press "Use this template" and it lands in your workspace as an editable form with all 4 questions already in it. No card, and nothing is published until you say so.

  2. Cut what you do not need

Every question you remove is one fewer reason to abandon the form. It takes about 1.5 minutes to fill in as it stands, and that is the number worth protecting.

  3. Add from the question bank

Pick what you are missing from the questions below. Drag any of them in, change the wording, and use conditional logic so people only see the ones that apply to them.

  4. Share it, or put it on your own site

Publish to get a link, or embed it in a page you already have. Answers arrive in your dashboard, and you can send them on to anywhere else with a webhook or an integration.

32 callback request form questions to choose from

A callback form is a promise with a time attached, and the whole thing turns on whether you can keep it. Asking when someone is free is the difference between one call that connects and four that go to voicemail.

Below: the questions that make a callback land first time, and the ones that qualify the call before anyone picks up the phone.

Reaching them

  • What is your name?
  • What number should we call?
  • Is that a mobile or a landline?
  • Which country is that number in?
  • What is your email, in case we cannot get through?
  • Which time zone are you in?
  • Is it alright to leave a voicemail?
  • Is there an extension or a direct line?

When to call

  • What day suits you best?
  • Morning, afternoon or evening?
  • What is the earliest we should call?
  • What is the latest?
  • Are there days that never work?
  • How soon do you need to speak to someone?
  • Would a shorter call today beat a longer one next week?
  • Is there anyone else who should be on the call?

What the call is about

  • What would you like to talk about?
  • Which product or service is this about?
  • Are you an existing customer?
  • Is this about a quote you have already had?
  • What is the main thing you need to know?
  • Is there anything we should look up before we call?
  • How did you hear about us?
  • Have you spoken to anyone here before?

Qualifying before you dial

  • What company are you with?
  • What is your role?
  • How many people would be using this?
  • Do you have a budget in mind?
  • When are you hoping to have something in place?
  • Are you looking at other options as well?
  • Who else is involved in the decision?
  • Is there anything that would rule us out?

Callback Request form questions, answered

What should a callback request form ask?
A number, a window when they will answer it, and a sentence about what the call is for. The window is the one people skip and the one that decides whether the call connects.
Should I ask for a preferred time or offer slots?
Offer slots if you can hold to them. A free-text 'when suits you' puts the scheduling work on your side and usually produces 'anytime', which means nothing. On the Pro plan a booking block shows real availability and the slot is actually held.
How quickly should I call back?
Within the window you promised on the form, and say what that window is before they submit. 'We call back within one working day' sets an expectation you can keep; saying nothing sets one you cannot.
Can the form book the call directly instead?
Yes. A booking block lets someone pick a real slot from your availability and holds it, so there is no callback to arrange. It is on the Pro plan and needs no external calendar tool.
Is this callback request form template free?
Yes. Use it, publish it and collect answers on the free plan with no card. Paid plans add things like payments, scheduling, removing our badge and your own domain, but none of them are needed to run this form.
Do I need an account to use it?
You need one to edit and publish, because the form has to live somewhere and the answers have to come back to you. Signing up takes about a minute and does not ask for a card. The people filling your form in never need an account.
Can I change the questions?
All of them. The template is a starting point, not a fixed form: add, remove, reword and reorder anything, change the look, and use logic so people only see the questions that apply to them.
Can I put it on my own website?
Yes. Embed it in a page you already have, or share the link. On a paid plan you can serve it from your own domain so the address says your name rather than ours.
Where do the answers go?
Into your dashboard, where you can read, search and export them as CSV. You can also have each response emailed to you, pushed to another tool with a webhook, or sent to a connected integration.
How long does this form take to fill in?
About 1.5 minutes as it stands, across 4 questions. Every question you cut brings that down, and shorter forms get finished more often.
