How to use this template
Open it as a draft
Press "Use this template" and it lands in your workspace as an editable form with all 2 questions already in it. No card, and nothing is published until you say so.
Cut what you do not need
Every question you remove is one fewer reason to abandon the form. It takes about 1 minutes to fill in as it stands, and that is the number worth protecting.
Add from the question bank
Pick what you are missing from the questions below. Drag any of them in, change the wording, and use conditional logic so people only see the ones that apply to them.
Share it, or put it on your own site
Publish to get a link, or embed it in a page you already have. Answers arrive in your dashboard, and you can send them on to anywhere else with a webhook or an integration.
32 cancellation survey form questions to choose from
A cancellation survey has a few seconds of attention and a person who has already decided. Make it one question with the option to say more, and put the save offer after the answer rather than in front of it.
The reason codes matter more than the prose: they are what you can count.
The reason
- Why are you cancelling? (Pick the closest)
- Too expensive for what we use
- Missing something we need
- Too difficult to use
- Switching to something else
- No longer need it
- Had a bad experience
- Something else
Digging in
- What specifically was missing?
- What were you trying to do that you could not?
- What are you moving to, if anything?
- What does it do better?
- What price would have felt right?
- Was there a particular moment that decided it?
- Did you contact us about it?
- If you did, how did that go?
Could it have gone differently
- Is there anything we could have done?
- Would a different plan have worked?
- Would a pause rather than a cancellation suit you better?
- Would more help with setup have changed things?
- Did you get enough value to justify the cost at any point?
- How long did it take to decide?
- Would you consider coming back later?
- What would bring you back?
Before they go
- How long were you with us?
- Which plan were you on?
- How often did you use it?
- Who else at your company used it?
- Would you recommend us to someone it suited better? (0 to 10)
- Do you need help exporting your data?
- Would you like to hear from us if this changes?
- Anything else you want to tell us?
Cancellation Survey form questions, answered
- What should a cancellation survey ask?
- One multiple-choice reason, because that is what you can count, and one open box for the detail. Anything longer gets skipped by the people whose reasons you most need.
- Should I make the cancellation survey required?
- No. Blocking a cancellation behind a form turns a quiet churn into a complaint, and the answers you extract that way are not honest anyway.
- Should I offer a discount to stop someone cancelling?
- Only after they have told you why, and only when price was the reason. Offering money to somebody leaving because the product is missing a feature reads as not listening.
- How do I use cancellation reasons?
- Count them monthly and watch the mix change. Any single reason is a story; the shift in proportions is the signal, and it usually moves a quarter before revenue does.
- Is this cancellation survey form template free?
- Yes. Use it, publish it and collect answers on the free plan with no card. Paid plans add things like payments, scheduling, removing our badge and your own domain, but none of them are needed to run this form.
- Do I need an account to use it?
- You need one to edit and publish, because the form has to live somewhere and the answers have to come back to you. Signing up takes about a minute and does not ask for a card. The people filling your form in never need an account.
- Can I change the questions?
- All of them. The template is a starting point, not a fixed form: add, remove, reword and reorder anything, change the look, and use logic so people only see the questions that apply to them.
- Can I put it on my own website?
- Yes. Embed it in a page you already have, or share the link. On a paid plan you can serve it from your own domain so the address says your name rather than ours.
- Where do the answers go?
- Into your dashboard, where you can read, search and export them as CSV. You can also have each response emailed to you, pushed to another tool with a webhook, or sent to a connected integration.
- How long does this form take to fill in?
- About 1 minutes as it stands, across 2 questions. Every question you cut brings that down, and shorter forms get finished more often.
