Community survey

Demographics kept to what's actually needed, priorities ranked, and one open question. Collecting demographic data you won't use is a liability, not an asset.

Worth knowing

  • The welcome screen and the intro text above the first question carry example turnaround times, policies and promises. Rewrite them as yours before you publish — they are the part respondents take literally.

How to use this template

  1. Open it as a draft

Press "Use this template" and it lands in your workspace as an editable form with all 4 questions already in it. No card, and nothing is published until you say so.

  2. Cut what you do not need

Every question you remove is one fewer reason to abandon the form. It takes about 2 minutes to fill in as it stands, and that is the number worth protecting.

  3. Add from the question bank

Pick what you are missing from the questions below. Drag any of them in, change the wording, and use conditional logic so people only see the ones that apply to them.

  4. Share it, or put it on your own site

Publish to get a link, or embed it in a page you already have. Answers arrive in your dashboard, and you can send them on to anywhere else with a webhook or an integration.

32 community survey form questions to choose from

Community surveys get read by people who will notice if you ignore them. Publish the results, including what you are not going to do, or expect the next one to go unanswered.

Keep the demographics coarse. A small community can identify someone from three specific answers.

How they take part

  • How long have you been part of this community?
  • How often do you take part?
  • How do you usually participate? (Reading, posting, events, volunteering)
  • What first brought you here?
  • What keeps you here?
  • Which activities do you value most?
  • Which do you never use?
  • Has your involvement changed recently?

What is working

  • What do we do well?
  • What has been the highlight of the past year?
  • Do you feel welcome here?
  • Do you feel heard?
  • Is it easy to find out what is happening?
  • Are the rules clear and fairly applied?
  • Would you recommend joining to a friend? (0 to 10)
  • Have you made a connection here that mattered?

What to change

  • What is the one thing we should change?
  • What should we stop doing?
  • What is missing?
  • What would get you more involved?
  • What puts you off taking part?
  • What should we spend money on?
  • When and where should events be held?
  • What would make this more welcoming to new people?

Helping out

  • Would you like to volunteer?
  • What could you help with?
  • How much time could you give?
  • Would you help organise an event?
  • Would you be willing to join the committee?
  • Do you have a skill we could call on?
  • What is your email address, if you would like to be contacted?
  • Anything else you want to say?

Community Survey form questions, answered

What should a community survey ask?
How people take part, what they value, and one open question about what to change. Then publish what you heard. A community survey whose results never appear is the last one people answer properly.
How do I get more responses from a community survey?
Keep it short, say how long it takes, say what it is for, and send a reminder once. Telling people what changed because of the last one does more than any incentive.
Should a community survey be anonymous?
Usually yes, and be careful with demographics. In a group of two hundred, age bracket plus role plus how long someone has been a member can identify a single person.
How do I report community survey results?
Share the numbers, the main themes, and what you will and will not act on. The 'will not' section is what makes the rest credible.
Is this community survey form template free?
Yes. Use it, publish it and collect answers on the free plan with no card. Paid plans add things like payments, scheduling, removing our badge and your own domain, but none of them are needed to run this form.
Do I need an account to use it?
You need one to edit and publish, because the form has to live somewhere and the answers have to come back to you. Signing up takes about a minute and does not ask for a card. The people filling your form in never need an account.
Can I change the questions?
All of them. The template is a starting point, not a fixed form: add, remove, reword and reorder anything, change the look, and use logic so people only see the questions that apply to them.
Can I put it on my own website?
Yes. Embed it in a page you already have, or share the link. On a paid plan you can serve it from your own domain so the address says your name rather than ours.
Where do the answers go?
Into your dashboard, where you can read, search and export them as CSV. You can also have each response emailed to you, pushed to another tool with a webhook, or sent to a connected integration.
How long does this form take to fill in?
About 2 minutes as it stands, across 4 questions. Every question you cut brings that down, and shorter forms get finished more often.
