A whistleblowing form only works if people believe it is anonymous. That belief is built by what the form does not ask: no name field, no email requirement, no login.
Say plainly what is recorded and what is not. Vague reassurance reads as evasion, and somebody weighing up whether to report will notice.

Anonymous Reporting form questions, answered
- How do I make a whistleblowing form genuinely anonymous?
- Do not ask for a name or require an email, do not put it behind a login, and turn off anything that records an identifier. Then say exactly that on the form, because people assess anonymity from what you ask rather than what you promise.
- Does the form record IP addresses?
- Published forms set no cookies and do not store a raw IP address against a response. Say what you do record, in plain words, on the form itself.
- How can we follow up on an anonymous report?
- Offer an optional contact method and explain exactly who would see it. Some reporters will give one if they understand the limits, and the option costs nothing to those who will not.
- Who should receive whistleblowing reports?
- A small named group, usually outside the normal management chain, and ideally including someone independent. Restrict access in the workspace so the report does not reach the person it is about.
- Is this anonymous reporting form template free?
