What publishing does

Publishing takes a complete copy of the form as it is at that moment and stores it, unchanged, forever. That copy is what the public URL serves.

Every response records which version produced it. That's why a response from six months ago still shows the questions as they were then, including ones you've since deleted.

Rolling back

Rolling back to an earlier version doesn't rewind history — it copies that version's questions into your draft and publishes them as a new version. Responses pointing at the old one stay exactly as they were.

Deleting a question

Deleting a question that already has answers hides it from the form but keeps the answers. They appear in the response view greyed out and marked as removed, so a historical response is never silently incomplete.
